New patient registration
You can register at the practice by completing and signing a registration form available from reception, or you can also download our Registration Form.
Please ensure every page of the document is completed correctly, we will not accept incomplete forms. For details of how to fill correctly complete the form, please see this guide.
If you choose to complete the form online, there are a number of ways of getting it back to us, you can;
* Print and complete the form and hand it in at reception.
*Post the completed form to us at Bellevue Group Practice, Bellevue Terrace, Newport, NP20 2WQ.
*Or, if you have the facilities, you can print and complete the form, then scan and email it to Bellevue.Enquiries@wales.nhs.uk
Please contact us for further queries, but bear in mind the registration process can take up to 48 hours from us receiving your completed form.
Please note that we only register patients who are entitled to receive NHS treatment and whose permanent address lies within our catchment area.
Once you have registered, please fell free to ask at reception for a patient information leaflet, or download a web-friendly one here.
Please note that measures in place due to the pandemic response mean we are currently only able accept new patients who have moved into our catchment area.
